Willis动脉环变异与颅内动脉瘤关系的临床探讨(论文3900字)
摘要:目的 探究在数字血管造影的诊断下大脑Willis动脉环的变异情况与颅内动脉瘤发生的关系。方法 分析2010年1月至2014年12月我院收治的144例颅内动脉瘤患者的数字血管造影资料,根据所有患者动脉瘤所在部位,将患者分为前交通动脉瘤组和后交通动脉瘤组,比较两组患者的性别、年龄、颅内动脉瘤瘤体的长径情况,探讨颅内动脉瘤与willis环分型的关系。结果 通过数字血管造影共检出144例颅内动脉瘤患者,其中前交通动脉瘤患者68例,后交通动脉瘤患者76例;前交通动脉瘤组患者与后交通动脉瘤组患者在性别分布、年龄分布、瘤体长径并无明显差异;前交通动脉瘤组中,A1段纤细或缺如率达79.46%,而后交通动脉瘤组中A1段纤细或缺如率仅为52.73%,两组比较有统计学意义(P<0.05);前交通动脉瘤组中双侧胚胎型大脑后动脉为62.47%,后交通动脉瘤组中右侧或左侧(单侧)胚胎型大脑后动脉为58.95%(P<0.05)。结论 经数字血管造影,能够较为准确的诊断出颅内动脉瘤;前交通动脉瘤的发生与A1段纤细、缺如以及双侧胚胎型大脑后动脉关系较为紧密,而后交通动脉瘤的发生与单侧胚胎型大脑后动脉的关系更为密切。

Clinical study of relationship between Willis arterial loop variation and intracranial aneurysms
Abstract: Objective  To explore the relationship between the changes of the cerebral arterial circle of Willis and the occurrence of intracranial aneurysms in the diagnosis of digital angiography.Methods  From December 2014 to January 2010, 144 cases of intracranial aneurysms were treated in our hospital,according to the location of the aneurysm of all patients,the patients were divided into the anterior communicating artery and the posterior communicating artery aneurysms,the relationship between the two groups of patients were compared by the sex, age, and the length of the intracranial aneurysms, and the relationship between the Willis ring and the intracranial aneurysms.Results  A total of 144 cases of intracranial aneurysms were detected by digital angiography,among the 68 patients with anterior communicating artery aneurysms,posterior communicating artery aneurysm in 76 cases;There were no significant differences in sex distribution, age distribution and length of tumor in the patients with anterior communicating artery aneurysms and the patients with a group of patients with a posterior communicating artery aneurysm;The anterior communicating artery aneurysm group, A1 segment of slender or absence rate up to 79.46% and communicating artery aneurysm group A1 segment of slender or absence rate is only 52.7%, two groups compared with statistical significance (P < 0.05);In the anterior communicating artery, the posterior cerebral artery was 62.47%, and the right and left (unilateral) embryonic cerebral artery was 58.95%in the middle cerebral artery (P<0.05) .Conclusion  Digital angiography can be more accurate in the diagnosis of intracranial aneurysms;Anterior communicating artery aneurysm occurrence and A1 segment of slender, absent and bilateral embryo brain artery relationship more closely and communicating artery aneurysms and unilateral embryonic brain artery relationship more closely.
